This policy is prepared in accordance with the Privacy Act 1988 (Cth).
Information we may collect at Purple Patch Child Therapy
Details such as name, address, contact details and date of birth
Personal enquiries, including presenting concerns, symptoms and therapeutic goals
NDIS information, including NDIS number, plan details and plan manager information
Appointment and billing details (including card details for clients who have selected automatic payments)
Contact details of other practitioners or parties involved in your care team (e.g. social workers, schools)
Court orders, including but not limited to parenting agreements or family violence orders
Additional health information such as referrals, reports from other professionals, diagnoses, family history and prescriptions
Any other information reasonably necessary to provide our services
We may collect this information from:
You directly, via phone, face-to-face sessions, email, intake forms or our online enquiry form
A person responsible for you, such as a parent or legal guardian
A third party, where permitted or required under the Privacy Act 1988 (Cth) or other relevant law. This may include practitioners within your care team, specialists, schools or plan managers
Storage of information
Your information is stored securely using Halaxy which utilises encrypted servers. Access is restricted and protected by two-factor authentication.

Purposes for collection, use and disclosure
We collect, hold, use and disclose personal information for the purposes of:
Providing therapeutic services
Communicating with you, a parent or legal guardian, or an approved third party regarding services provided
Consulting with professionals involved in your care, such as schools, allied health providers or social workers
Managing accounts and billing, including communication with NDIS plan managers and processing automatic payments
Responding to feedback or formal complaints
Advising clients of important updates or events related to our practice
Confidentiality and disclosure of information
All client information, including case records, is confidential, securely stored, and will only be disclosed under the following circumstances:
For individual or group supervision and professional development purposes. In these situations, identifying details are removed or altered to protect client confidentiality
Where the therapist determines there is a serious risk of harm to the client or another person
Where abuse is disclosed, or there are reasonable grounds to suspect a child has been abused, is at risk of abuse, ill-treatment, or exposure to psychologically harmful behaviour
Where disclosure is required by law, including by court order or under the Freedom of Information Act
Where written consent has been provided authorising communication with another professional, person or organisation
Practitioners aim to communicate openly and transparently with parents and caregivers about the therapeutic process and progress, while also respecting the child’s right to privacy. Practitioners retain professional discretion regarding the timing and extent of information shared, guided by the child’s best interests and therapeutic needs.
